This Featured Exhibitor Opportunity is on a first come, first served basis. Before submitting this form, the following requirements must be met:

1. Company’s Certificate of Insurance must be submitted to Beverly Elliott, CEM and it has been approved.
2. Company’s booth has been paid in full.

Note: If either requirement is not met, you will not be considered for this opportunity.
